hoc facite quotiescumque sumitis, in meam commemorationem.

German translation

(Das ist) der Leib, der für euch hingegeben wird:
Das ist der Kelch des neuen Bundes in meinem Blut, spricht der Herr:
Tut dies, sooft ihr davon nehmt, zu meinem Gedächtnis.

This is my body which is given for you;
